Change of Address
It is very important that we hold a correct address and telephone number for you. If either of these change please notify us.
Register with our practice
Patients living in the G41 postcode area (except G41 1) wishing to register with our Surgery should come to the surgery to complete a registration form. We ask that you bring 2 forms of ID with you, one with your photograph and one with your address in the area.
Repeat Prescriptions
Our patients are able to order repeat prescription items online. These items are listed on the right hand side of your repeat prescription computer slip.
Meet Your Midwife
Expecting a baby? It’s never ‘too early’ in a pregnancy to register for maternity services.
As soon as you have had a positive pregnancy test, we recommend you start taking Folic Acid and Vitamin D supplements. Your local pharmacy can advise you on how much to take.
